Monday through Friday - 8:00 am to 5:00 pm
Insurance/Payment:
The physicians at the Eye Center of St. Augustine participate with Medicare and most major medical health insurance plans. Managed care patients who require prior authorization or a referral are asked to obtain these prior to their visit. We ask that patients pay for non-covered services, such as eye refractions, and co-payments at the time of their visit.
Eye Center of St. Augustine Accepts the Following Insurance Plans:

- Blue Cross Blue Shield
- United Healthcare
- Cigna
- Aetna
Visa and Master Card are accepted. Additional financing options exist through Care Credit.

Cancellation/No-Show Policy:
We require a 24 hour notice for cancellations. Failure to give notice of cancellation will be noted as a “no-show.” There is a fee of $25.00 for “no-show” appointments.
Appointment Checklist :
- Completed Paperwork
- Photo ID
- Insurance Card(s) (Note: If your insurance requires a referral, please bring your referral from your Primary Care Physician with you.)
- A list of all medications you are currently taking, including eye drops
- Your eyeglasses
- If you wear contact lenses, please bring your current prescription (including brand, power and base curve).
- Sunglasses (If you are coming in for a routine eye examination, your eyes will be dilated and will be sensitive to light.)
